BS EN 207 / AOR 2010 — Laser & IPL Site Audit Data Entry Form — generates a ready-to-run audit report script

How to use: Fill in all sections below. Required fields are marked *. Use Save to preserve your work to the shared server (if connected). Use Generate Report to produce the .docx. To transfer an audit between machines or create a portable backup, use Export Audit / Import Audit on the Settings tab.
Document Metadata

Laser Protection Supervisor (LPS)
Authorised Persons (Device Operators)
Enter each person authorised to operate laser / IPL devices. Up to 6 persons. Leave unused rows blank.
Clinic Details
Each room or area where laser/IPL devices are used. Devices can be assigned to a specific LCA in the Systems tab. If the clinic has only one treatment area, one entry is sufficient.
Name of the local authority. Used in the audit report and notification advisory letter.
Issue number shown on the Local Rules cover page.

Laser & IPL Systems
Enter up to 6 systems. Set type to "(Not used)" for unused slots. If a device operates at more than one wavelength or mode (e.g. a dual-wavelength Nd:YAG at 1064 nm and 532 nm), enter each configuration as a separate system tab.

    HSE 5-steps device-specific risk assessment for each active system. One section per device in a single document.
    👥 Register of Authorised Users
    Per-device register of authorised operators with designation, training date, and signature columns. One section per device.
    ✅ Start-Up Check List
    Single-page laminated laser/IPL start-up checklist for display in the treatment room. Clinic name and address replace the header; LPS contact in warning line.
    ⚠️ Notice to Persons
    Single-page notice for display in the controlled area. Clinic name and address in the header; controlled area name inserted in the title; "NO" highlighted in red.
    📡 Monitoring Programme
    E7 — Optical Radiation Monitoring Programme. AOR 2010 worker exposure assessment, worker classification, and health surveillance determination.
    👷 Worker Classification Record
    E8 — AOR 2010 worker classification record. Identifies workers at risk, documents the exposure assessment basis, records the classification outcome (below/above EAVs), and determines Regulation 5 health surveillance obligations.
    📁 Generate Final Docs
    Saves the full audit pack to a chosen folder: Audit Report, Notification Letter, Local Rules, Device Risk Reports, Authorised Users Register, Cover Letter, Start-Up Check List, Notice to Persons, Monitoring Programme, Worker Classification Record, and Audit Data JSON. Optional: Safety Policy and Training Programme. Status forced to Issued; linking notes excluded.

    These documents are not generated automatically as part of the LPA site visit. They are standalone deliverables that draw on the same audit data.

    Laser/ILS Safety Policies & Procedures document. Covers electrical, ocular, LGAC, flammability, access control, shutdown, review, incidents, audit sheets, skills validation, and roles.
    📚 Training Programme
    Laser/IPL Safety Training Programme. Four-stage pathway: CoK, device-specific, preceptorship, CPD. Includes training check sheets and CPD log.
    🚨 Incident Investigation SOP
    Laser/IPL incident and near-miss investigation SOP. Covers immediate response, root cause analysis, corrective actions, MHRA Yellow Card reporting, and review cycle.
    These settings apply to all audit reports generated on this device. They are pre-filled with GCG defaults and rarely need to change.
    ► Backup / Portability (Export & Import)
    Settings (LPA name, organisation, checklist descriptions) — reusable across all clinic audits.
    Audit data (clinic details, devices, checklist results, findings) — saved per clinic/audit. Import a clinic profile to pre-populate the form.

    Use Import Audit to restore a previously saved audit JSON, or to load an audit onto a different machine. Export Audit creates a standalone JSON copy — useful as a manual backup or for archiving. Note: 💾 Save on the Audit tab already downloads a JSON file automatically.

    Optional — appears on sign-off sections of generated documents
    LPA Profiles
    Save the current LPA settings (including signature) as a named profile. Switch profiles when generating documents for different LPAs.
    Shared Server
    Connect to the LPA audit server on your NAS to share audits with other users in real time. Leave blank to work offline (local save only).

    Identify an unknown laser or IPL device using web search and/or image analysis, then populate its specifications directly into System 1 on the Audit tab. Tip: photograph the device's rating plate — it usually contains the model number, CE mark, and manufacturer even when the front panel branding is generic.
